The obesity epidemic is a worldwide reality. It is defined by the World Health
Organization as abnormal or excessive accumulation of body fat that can reach degrees
capable of affecting health. Diabetes mellitus, hypertension, and dyslipidemia are some
examples of diseases associated with obesity. Obesity is related to an increased risk of
chronic kidney disease and progression to end-stage renal disease (ESRD), as well as an
increase in mortality from cardiovascular and non-cardiovascular causes. In obese patients
on hemodialysis, there is the phenomenon of the obesity paradox, in which obese patients
have an increase in survival over non-obese patients. Peritoneal dialysis is associated with
increased prevalence of the metabolic syndrome, but the prognosis remains uncertain. The
impact of obesity on kidney transplantation is undefined, but there appears to be no increase
in mortality. The treatment of obesity in patients with ESRD is a challenge and involves a
multidisciplinary approach. These and other aspects of obesity and renal replacement
therapy will be discussed in this chapter.
