Abstract:
The application of established physical principles to dialysis – specifically mass
conservation and mass transport - reveals the dynamics of manipulation of human
physiology that result from this therapy. Correction of acid base is used as an application
of these principles in end-stage renal disease (ESRD); this analysis spotlights the dramatic
impact of trying to reverse the acidification of body buffers that have had to sequester H+
over a 2 – 3 day period during a 3 – 4 hour treatment. An additional finding is that fluid
management in the ESRD patient has a major impact on the ability to reverse acidification
due to the significant blunting of base uptake by ultrafiltration.
