This book is composed of thirteen chapters. Chapter 1 provides a short overview of the Air Transport System, from both the micro- and macro-level structure. It covers the descriptions and definitions of the main system elements: air carriers, airports and air navigation service providers, as well as personnel, equipment, procedures and the environment. Chapter 2 introduces the reader to the basic concepts in air transport risk and safety. This chapter covers the definitions of safety, hazards, risk, incidents and accidents. It further explains safety criteria, safety barriers, safety regulatory requirements, and finally it compares Safety I and Safety II concepts. Chapter 3 covers the field of Air Transport Safety Metrics and Records. Here safety metrics, accident statistics and safety records are explained and illustrated. Finally, a safety comparison of transport modes is made. Chapter 4 presents Sources of Accident/Incident Information. Explained here is how safety-related events (incidents and accidents) are investigated and what the phases of the investigation process are, as well as how safety information is collected. Chapter 5 describes the main safety issues in contemporary air transport. They are grouped into three sets: airport, air navigation service providers and air carriers safety issues. Chapter 6 covers the Safety Culture and Just Culture concepts and related issues, such as the components of safety culture, safety culture indicators, measurement and maturity, as well as the principles of just culture and its benefits. Chapter 7 focuses on the Human Factor and Threat and Error Management. It covers concepts such as human errors, performance, fatigue and fatigue management. Special emphasis is given to threat and error management (TEM) from the point of view of both air carriers (LOSA) and air navigation service providers (NOSS). Next, in Chapter 8, the concept of Safety Management Systems (SMS) is introduced. It covers the SMS process and framework, as well as the relation between SMS and quality management systems. Finally the state safety program (SSP) concept is introduced. Chapter 9 covers Safety and Risk Management. It explains safety management strategies, key activities and processes, followed by safety oversight and performance indicators and targets. Risk management covers hazard identification, risk assessment, mitigation and communication. Chapter 10 covers the concepts of Safety Oversight and Audit. Audits for air carriers, airports and air navigation service providers are emphasized in it. Chapter 11 presents the safety assessment methodologies describing different types of methods/models used. Generic safety assessment process is presented as well as NLR’s TOPAZ and Eurocontrol’s SAM methodologies. Special attention is given to the Hazard Identification Approaches. Chapter 12, titled Safety Thinking and Accident Modeling Approaches explains the evolution of safety thinking in the field of aviation and presents the main accident modeling approaches. Finally, Chapter 13 presents Safety Nets as a special part of the air transport system aimed at enhancing air transport safety.
